STEP 2:
INFORMATION YOU WILL NEED TO COMPLETE THE SESSION:

• Your Social Security number
• Your driver’s license number
• Name, address, and telephone number of your parents or guardian
• Name, address, and telephone numbers of two relatives other than your parents
• Name, address, and telephone numbers of two references, other than relatives, from your home locality
• Your FSA pin, if you wish to electronically sign your Master Promissory Note (MPN)

Perkins Exit Interviews

STEP 1:

• CONTACT A  FINANCIAL OPERATIONS REPRESENTATIVE AT (229) 430-3728 TO OBTAIN AN ACCOUNT NUMBER.

STEP 2:

• Click Here or go to https://www.mycampusloan.com.
• THEN CLICK ON FIRST-TIME USER BUTTON TO REGISTER FOR AN ACCOUNT.

STEP 3:

• CLICK ON E-EXIT BUTTON ON LEFT TO START EXIT INTERVIEW.

STEP 4:
INFORMATION YOU WILL NEED TO COMPLETE THE SESSION:

• Your Social Security number
• Your driver’s license number
• Name, address, and telephone number of your parents or guardian
• Name, address, and telephone numbers of two relatives other than your parents
• Name, address, and telephone numbers of two references, other than relatives, from your home locality
